Due to the announcement from Microsoft that the fade out of Internet Explorer (IE) and end of support of Microsoft Silverlight the HA CRER Portal will become obsolete by June 2022. Therefore, the Joint CREC-NTEC CREC will no longer accept submissions via the HA CRER Portal from 17 May 2022 until further notice.
With effect from 17 May 2022 please follow the submission procedures below:
For New/Initial Application
- Complete the attached CREC Application Form, Version 3, dated 30 March 2012 (Fillable PDF Form) or downloadable at http://www.crec.cuhk.edu.hk/download/
- Email the completed CREC Application Form (Fillable PDF Form) without signature to the CREC Office (crec@cuhk.edu.hk)
- Print out the submission email and enclose it with the submission package (i.e. the original signed version of the completed CREC Application Form and all relevant study documents) to the CREC Office, as per usual practice.*Please note that it is the Principal Investigator’s responsibility to ensure that the hardcopy and softcopy of the CREC Application Form is consistent. In the event of any inconsistency between the hardcopy and softcopy the hardcopy shall prevail.
For Amendments, Ethics Renewal/Completion and Protocol Deviations
- Complete relevant form which can be downloaded at http://www.crec.cuhk.edu.hk/download/
- Submit the original signed version of the completed form to the CREC Office with all relevant study documents, if applicable
For Correspondences and SUSARs
- Submit the original signed cover letter and relevant study documents to the CREC Office
During the grace period of now until 16 May 2022, submission either on the HA CRER Portal (current practice) or the above procedure is acceptable. Thereafter, submission on the CRER HA Portal is not accepted.
